HOPE serves those in our community who are 100% uninsured.
 
Patients are seen by appointment only.

Patients are asked to donate $5 - $15 to help cover the costs of services.  However, no patient is turned away for inability to pay.  


Enrollment:

All prospective patients seen at HOPE are required to complete an  application and meet with an Eligibility Specialist.
 
To determine eligibility, please call and speak with one of our Eligibility Specialists or come by the clinic.


Required documents:

Valid picture ID (original - we will make a copy)

Proof of household income (from past 3 months)

Proof of address (a bill in your name with address)
